General Information

Financial Policy

The fee for your endodontic treatment will be based on the extent of treatment. During your first visit we will discuss the probable number of visits, their lengths, and the fees involved.

If we only provide Endodontic Evaluation (Limited Evaluation, Consultation):

     This consists of an examination and testing, imaging, discussing the likelihood of maintaining the tooth and treatment options available to you.  Payment is due at the time of service.  

If we provide Treatment:

     Our office is committed to providing you with the best endodontic care possible. Due to the unique nature of our practice, our financial polices are significantly different from other dental practices. The following is a statement of our financial policies. 

Payment for treatment is due at the time the treatment is rendered. For your convenience we accept cash, personal checks, Visa, MasterCard, Discover, and American Express. Endodontic care will usually range in between $600.00 and $2,000.00. Dr. Mallick's team will review all of your anticipated treatment fees with you prior to doing any treatment.

Our office is not contracted with any insurance companies. If you have dental insurance, as a courtesy our office will help you file your insurance claim for reimbursement to you. Please bring all necessary information on your insurance policy. Please remember your insurance policy is a contract between you, your employer, and the insurance company. All charges are your responsibility whether your insurance company reimburses you or not. Please remember to update our office regarding any changes to your dental insurance policy to avoid any delays in processing your claim.

Check payments are happily accepted.  However, if check payment is returned for any reason, an additional $50.00 fee will be added to your statement.  This is to cover the returned check fees that the office will ensue from the bank.

Scheduling

We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.

We try our best to stay on schedule to minimize your waiting. Due to the fact that our practice provides many types of treatment, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

 

Cancellations/Rescheduling

Your scheduled visit is reserved especially for you. If you need to cancel or reschedule your appointment, please call us at least 24 hours prior to your appointment time. This allows us to accommodate other patients with endodontic needs or dental emergencies. Thank you for your cooperation.
